Data Accreditation and Improvement Incentive Scheme

Supporting GP practices to provide better care and understand the wider problems affecting patient health.

The Data Accreditation and Improvement Incentive Scheme is jointly led by CEG and NHS North East London, with support from EQUIP and Edenbridge.

The scheme is helping practices to record high-quality data in two areas:

  • Consultation types and job roles – to identify pressure points by understanding who is doing what within the practice.
  • Non-medical needs of patients within the most deprived quintile of the North East London population – to enable practice teams to understand the needs of this cohort, tailor health recommendations, and signpost to local services within the community.

Patient video: Asking about the wider determinants of health

This video is available to download from the list of resources above. 

Available offline / in your clinical system:

  • Accurx Florey - Send questions about the wider determinants of health to patients by text message. Patient responses are coded into their health record automatically. We have sent the Florey import link by email to North East London practices - please contact your CEG facilitator if you have not received it.
  • Support searches - Identify patients defined by the scheme as 'at risk' (living in deprived areas which are likely to have high levels of need) and all new registrants since 1 October 2023. These patients can be prioritised to receive the text message, though some practices may prefer to send the message to a wider group.
  • Registration document: 'Wider determinants of health questionnaire.' - Includes the wider determinants questions so you can ask these routinely when registering new patients. Available in EMIS (Resource Publisher) and SystmOne across North East London.
  • Template: 'Wider Determinants of Health NEL'- Ask patients the required questions during a consultation. Available as a page within all CEG templates and as a standalone template in EMIS (Resource Publisher) and SystmOne across North East London.
