The Asthma Prescribing Tool (APT) is a related tool designed to assist with the identification and management of patients who are at higher risk of hospital admission due to their asthma.

The APT and related documentation are available from the REAL-HEATH Respiratory page

The Active Patient Link (APL) tools are practice facing clinical tools that identify patients with a practice with a particular condition (e.g. Atrial fibrillation or cardiovascular disease) and allow healthcare professionals to perform a virtual patient review and clinical audit.

Click on a banner below for specific information about one of the APL-Tools.

The APL-Tools are freely available.  If you would like to obtain a copy, please use our APL-Tools Request Form.

Additional support for the APL-Tools is only available to CEG member practices.

For any further information, please contact us by emailing ihse-ceg-admin@qmul.ac.uk.
