CEG Clinical Template Available

  • COVID-19 template provides one place for recording consultation data for possible, probable, confirmed COVID-19 cases and provides helpful links to assist with management of the case and public health matters.

  • Med 3 'Fit Note' for COVID-19 
  • Travel Insurance COVID-19
  • Cremation Form 4 Medical Certificate 
  • Health Protection Unit HPU Notification Form
  • Letter to patients who need to shield (very high risk)
  • Letter to patients who were wrongly coded as very high risk

CEG Protocols Available

  • Protocol available for EMIS and SystmOne that automatically codes "Provision of advice, assessment or treatment limited to COVID-19 pandemic" and enters the following text into a patient's record "Consultation carried out during time of COVID-19 Pandemic.  Management decisions taken in line with necessity and NHS advice at this time which may be outwith normal guidance".
  • Specific protocol for WEL practices that gives advice when referring to secondary care.
