REAL-HEALTH Cardiovascular
The REAL-HEALTH Cardiovascular initiative supports practices to improve their patients’ cardiovascular health. The programme provides guidelines, educational material and sessions, and useful audit tools. We are initially focusing on three aspects of cardiovascular health:
- Blood pressure control;
- High-intensity statin use;
- Reducing gastrointestinal bleeding in patients on antithrombotic treatment.

In-practice tool: 'APL-CVD'
We have developed an Active Patient Link (APL) tool that is used within the GP practice. It draws data from the patient records held within the practice and enables a 'virtual patient review'. APL-CVD helps practice teams identify patients with cardiovascular disease who may not be optimally treated, and supports medication review.
