In collaboration with UCL Partners, CEG has developed a set of "Search and risk stratification tools" which scan Electronic Patient Records and stratify patients with long-term conditions into risk groups.

The conditions include:

• Asthma
• Chronic obstructive pulmonary disease (COPD)
• Type 2 diabetes
• Atrial Fibrillation
• Hypertension
• Primary and secondary prevention of cardiovascular disease using statins

Typically, the searches divide patients into high, medium and lower-risk groups – with the high-risk groups managed by their GP and/or pharmacists, and the lower-risk groups by healthcare assistants where appropriate.

The searches are available for EMIS and SystmOne via the UCLP website, along with educational and supporting materials.
