Our Active Patient Link (APL) tools display information from patient records held within the GP practice, with filters to help practice teams review patients and prioritise appointments.
Please note that APL-CVD (Cardiovascular Disease), APL-T2DM (diabetes) and APL-HF (Heart Failure) have been decommissioned and are no longer available.

How to access

APL-Imms
APL-Imms is helping GP teams to navigate the complex childhood immunisation schedule and protect more children with routine vaccinations on time.
How it works
Practices run a single search in their clinical system and import the results into the tool to:
- Prioritise - List all patients aged 0-5 and filter them according to when their immunisations are due or overdue. Export lists in a range of formats, to send appointment messages or agree actions with colleagues.
- Tailor their approach - See relevant details about each patient in the list, such as contraindications and declines, simply by clicking on their name in the tool. Access information on vaccine ingredients and schedules to help guide conversations with parents.

APL-Imms is available free of charge to GP practice teams in the North East London NHS region. If you have not yet downloaded this tool, or need support using it, please contact your local CEG facilitator.
At the moment, APL-Imms is only compatible with EMIS. A SystmOne version is in development.

APL-NDPP
Type 2 diabetes has a major impact on individual health and increases the risk of other conditions. Using APL-NDPP, GP practice teams can quickly list patients registered with them who are eligible for behaviour change programmes through the NHS Diabetes Prevention Programme and prepare a prioritised list of people to invite.
How it works
Practices run a single search in their clinical system and import the results into the tool to:
- List all eligible patients
List all patients at risk from type-2 diabetes who may be eligible for the Diabetes Prevention Programme. Export a filtered list to send invitations for referral. - Prioritise
Rank patients according to the number and importance of their relevant risk factors, and filter according to whether they have already been referred or not, or have declined referral. - Tailor their approach
See relevant details for each patient in the list without leaving the tool - including age, vulnerabilities and language spoken.

Cancer Diagnosis Audit Tool
The Cancer Diagnosis Audit Tool supports GPs to reflect on past cancer diagnoses by quarter and identify any avoidable delays. The tool auto-fills fields using data from the patient record, making it easier to complete an audit and review delays in cancer care.
How it works
Practices run a single search in their clinical system and import the results into the tool to:
- Review quarterly activity - Audit cancer diagnoses one quarter at a time, or for multiple quarters in the previous financial year.
- Work smarter - The tool auto-fills audit fields using data from patient records
- Address inequalities - Reflect on patient demographics, co-morbidities, age at diagnosis, tumour group and identify any areas for improvement.

CEG software tools are available free of charge. APL-AF is available nationally, but user support is only provided for practices in the NHS North East London region. All other tools are restricted to North East London only - practice teams in the region should contact their local CEG facilitator for access.
